Our staff will convey a phone screening and send you a screening questionnaire. Please fill in the form and bring to your visit.

At your appointment
Please check in when you arrive by calling 978-694-1090 and wait in your car. Our office will call you once your room is ready.
We will check all visitor's temperature using touchless thermometer.
After your visit
Following America Dental Association (ADA) and Center for Disease Control (CDC) guidelines, our office will call you 48 hours after your visit to follow up if you have any COVID-19 symptom.
